You’ll join a supportive adult social care team working with adults with a range of needs to promote independence, wellbeing and positive outcomes. The team values collaborative, strengths‑based practice and reflective working, with regular supervision and peer support. You’ll be supported by experienced colleagues in an environment that encourages learning, professional development and high‑quality, person‑centred practice.

About the Role

As a Social Care Practitioner, you’ll manage a diverse caseload and work with a high degree of autonomy, supported by an experienced Team Manager. This is a role where your expertise and initiative will be valued—and where your impact will be visible.

Your responsibilities will include:

  • Leading adult care assessments and reviews under the Care Act 2014
  • Designing personalised care and support plans
  • Conducting Mental Capacity Assessments and Best Interests Decisions
  • Managing risk through thoughtful, person-centred strategies
  • Completing Continuing Health Care Checklists

About You

We’re looking for someone who is a motivated and compassionate practitioner with experience of working with adults who have a range of needs. You bring a strengths‑based, person‑centred approach to your practice and are committed to promoting independence and wellbeing.

You work well as part of a team, value reflective practice, and are confident in managing your workload while maintaining high professional standards. You are keen to learn, develop your skills, and contribute positively to a supportive team environment.

You’ll need:

  • Relevant qualification equivalent to NVQ Level 3 or equivalent Health and Social Care experience
  • Experience supporting individuals with autism, mental health conditions, learning disabilities, or physical disabilities
  • A Full UK Driving Licence and access to a vehicle
