About the Role
The role of a Home from Hospital Service Officer supports Bury residents aged 50+ helping them to re-adjust to living independently back at home, thereby reducing the need for re-admission to hospital, and reduce their time in hospital. You will report to the Home from Hospital Assistance Service Manager.
All staff will be subject to a DBS (Disclosure and Barring Service) check to verify their record status. Training on Safeguarding Vulnerable Adults and First Aid will be provided.
The Age UK office is based at the Jubilee Centre in Moorside, Bury. The Centre is situated in a park and overlooks the beautiful Clarence Park Lido. However, this role will be based at Fairfield General Hospital, Bury although may be required to work at other locations within the borough.

Key Responsibilities
- Support individuals within 48 hours of hospital discharge or referral
- Assess needs and develop tailored, person-centred support plans to promote recovery, independence and wellbeing
- Help overcome barriers that may delay discharge using a strengths-based approach
- Provide emotional support through reassurance, encouragement and active listening
- Deliver practical support such as shopping, meal preparation, collecting prescriptions and light domestic tasks
- Carry out home visits, assessments and regular telephone support
- Arrange transport and appointments, and liaise with health, social care and community services
- Signpost and refer to appropriate services including community groups, benefits advice and specialist support
- Maintain accurate records and provide regular updates to the Service Manager
- Attend training, team meetings and supervision, working flexibly to meet service needs
