Community Matron – Minehead Medical Centre

Location: Community-based – Minehead Medical Centre & patient homes
Hours: Part-time 20 hours available (Full-time 40 hours)

Minehead Medical Centre is seeking an experienced Community Matron to join our primary care team.

The Community Matron will support patients of all ages with diverse and often multifaceted conditions. They will provide personalised, holistic care for people with complex health needs across our community, with a significant focus on supporting our older and often frail population in Minehead and the surrounding communities. Consequently, the Community Matron will play a central role in supporting our patients to remain independent, avoid unnecessary hospital admissions, and receive high-quality care in the place they call home. This role is ideal for someone who is passionate about proactive, frailty-focused care and making a meaningful difference to the lives of older adults.

Key Responsibilities

Clinical Care:

  • Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions — including frail, elderly and housebound patients — in their homes, care homes and community healthcare settings.
  • Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
Work closely with families, carers and social care teams to support older patients with complex needs.

Leadership & Coordination:

  • Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
  • Support the development of proactive frailty identification and early intervention within the practice and PCN.

Full details of all responsibilities are provided in the Job Description.

About you - ideal candidate

You will be someone who enjoys building strong, trusting relationships with patients and their families. Given the needs of our local population, experience in supporting older adults — including frailty, geriatrics, care homes or complex long-term conditions — is highly valued.

Essential Qualifications and Experience

  • Registered Nurse with valid NMC registration
  • Independent Nurse Prescriber (V300)

Working at OneMedical Group

OneMedical Group is a nationally operating, family-owned healthcare organisation dedicated to building a healthier future for all. Since our founding in 2004 in Yorkshire, we have grown from a small, community-focused company to a leading provider of NHS primary care, urgent care, community services — and an innovator in healthcare property and estate solutions.

Our founding vision — by a committed father and daughter team — remains central: delivering care with compassion, integrity and a personal touch, like the traditional family doctor. Over two decades later, that founding spirit still guides our work.
