We have an excited opportunity for an experienced primary care Advanced Nurse Practitioenr looking to join our Shakespeare Urgent care team, working across our city centre walk-in centre and primary care streaming services co-located within the city’s hospitals.
You will be part of a team that are making a huge impact in supporting the urgent and emergency care system within Leeds, we the opportunity to influence how future urgent care services develop, and drive quality improvement, and work with a brilliant team.
You will be supported in your personal development as part of what is a new and innovative role, and receive dedicated management time including one offline day each month to support with the four pillars of advanced practice.
The successful candidate will work across multiple settings, giving the opportunity to work closely with different clinical peers, offering variety and a rich learning environment.
Working across Urgent Care Services in Leeds, you will manage acute presentations to the Walk-In Centre; in addition to using the clinical knowledge and skills to determine urgency and best pathway for patients presenting to the Emergency Departments in Leeds through our innovative primary care co-located service, which was nominated for a HSJ award in 2020 and received a high commendation.
As an experience Advanced Nurse/Clinical Practitioner you will utilise expert clinical and knowledge to manage patients presenting with acute, undifferentiated problems in addition to those with chronic long term conditions. You will assess, diagnose and plan care for patients; including prescribing of medications where qualified to do so, and where clinically indicated.
