About the Practice

St Thomas Road Surgery is based in South Derby near Peartree area, and cares for one of the most in need communities in the City. 
With a practice population of 4351 patients, we look to deliver the best patient outcomes and service, focused on promoting wellbeing, delivering planned personalised care, good patient continuity and access to the right care at the right time through innovation.
Located in Lister House Building, St Thomas Road, Derby, the location has good public transport links and on-site parking, and is close to the beautiful Derbyshire countryside.

Our practice population reflects the close knit community within which it sits. The majority of our patients are young, between the ages of 0 - 40, and whilst we have a lower prevalence of long term conditions than the local average, the impact of the wider and social determinants of health is significant. Promoting women and child health is an important part of our focus, along with managing conditions such as diabetes, hypertension and obesity.

Our patient demographic sits in the 1st most deprived decile and the majority of our patients come from non-white ethnic groups and multicultural backgrounds; working to care for this community is hugely rewarding and offers great variety. We play an integral role in educating and promoting public health messages to help our patients live healthier lives, working with the local community and our PCN. Given the demographic it is rare that home visits for patients are required.
What the team looks like:
We are made up of GPs, Advanced Nurse Practitioners Practice Nurses, with further support from our PCN including additional roles such as clinical pharmacist and social prescribing. Our non-clinician team is made up of our practice’s Business Manager, Clinical Service Manager, and Receptionist team, with support from Medical Secretaries and Clinical Coders working in a centralised hub. We also work closely with a sister service, the Derby Urgent Treatment Centre.

The Role
 The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients (adults and children) with undifferentiated and undiagnosed needs. The job holder will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making & planning of care and treatment, diagnosis, prescribing, health promotion & safe discharge of patients; will work in conjunction with multidisciplinary teams to provide holistic care and medical treatment to all patients & will have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.

Essential Qualifications & Training

Registered with relevant professional body
Degree-level registration qualification or equivalent post-registration award
MSc in Advanced Practice or equivalent qualification – or working towards/ willingness to undertake
PG Diploma in Advanced Practice
Non-medical prescribing
Life Support qualifications (or alternative profession specific clinically determined qualifications)
Relevant teaching qualification
