Riverside have a new opportunity available for a Journey Co-ordinator, working full-time, on a permanent basis.

Your salary will be £25,170 plus a location allowance of £4,000 per annum. Your working pattern will be on a rotational basis from Monday to Sunday.


Who you’ll be helping

Westminster Transitional housing approach, a brand new service model across 6 buildings in Marylebone.

Combination of 24 hour supported housing; 24 hour transitional housing (short term) and satellite housing led units to deliver strengths based support for single homeless customers. A team approach will work across all buildings making the most of technology and community spaces to develop best practice solutions to move on and meaningful occupation.


What will you do?

The delivery of individual, positive outcome focused support packages to Customers of Riversides Supported and Sheltered Schemes, ensuring that the support service promotes empowerment and independence. The role requires maintenance of a high level of specialised knowledge surrounding the specialism, acting as a centre of excellence in the specific specialism as well as a resource to colleagues for specialist knowledge, supporting colleagues to understand the latest developments in the field. In addition, you will oversee the customers’ pathway from referral to resettlement providing case management for the rest of the team and maintaining the overview of the customer journey to provide a seamless service regardless of the service the customer is in within the pathway.


Day to day

Your usual responsibilities as a Journey Co-ordinator will include:

• Co-ordinating the customer journey from start to finish and supporting the Journey Coaches in the development and delivery of the support/resettlement planning processes.
• Assisting Journey coaches to support Customers with day-to-day tenancy issues, payment of rent, claiming benefits.
• Undertaking referrals and assessments of potential Customers, working with them to identify the most appropriate housing solution for their needs.
• Ensuring Customers have access to relevant external support services.
• Working with Journey coaches to support Customers to agree and set goals and actions as well as meet meet clearly defined Customer outcome targets through key-working, support plans, risk assessments and other interventions.

Does this describe you?

You’re experienced in working with vulnerable customer groups as well as delivering structured support in either housing or social care. You have demonstrable IT skills including basic proficiency in Microsoft Office including Word and Excel. Although not essential, the ability to demonstrate an understanding of the PIE approach would be desirable.
