Due to ongoing growth and expansion, we are inviting Independent Prescribing (IP) Clinical Pharmacists to join our talent pool for future opportunities across our national PCN contracts. By joining our talent pool, you will be considered for full-time or part-time roles as they become available, aligned to your experience, availability and location.

About the Opportunity

As part of our talent pool, you may be considered for Clinical Pharmacist roles supporting GP practices within PCN settings. These roles offer the opportunity to work flexibly within GP core opening hours, while playing a vital role in improving patient outcomes and access to healthcare.

You would be supported by our experienced Senior Clinical Team, who provide ongoing training, development and workflow management within primary care environments.

Typical Responsibilities May Include

  • Working confidently within professional boundaries as part of a multidisciplinary team
  • Resolving medication-related queries from patients and practice staff
  • Conducting structured medication reviews (SMRs) and polypharmacy reviews
  • Undertaking telephone and long-term condition medication reviews
  • Medicines reconciliation post-discharge
  • Supporting medicines optimisation and quality improvement initiatives
  • Providing expert clinical advice on medication to clinicians and patients
  • Delivering audits and contributing to service improvement projects
  • Supporting QOF, CQC and local incentive scheme workstreams
  • Educating patients on medication use and disease prevention

What We’re Looking For

To be considered for our talent pool, you should have:

  • A minimum of 12 months’ experience working in general practice or primary care
  • GPhC registration as a Clinical Pharmacist
  • Independent Prescribing qualification
  • Experience using EMIS or SystmOne
  • ​​​​​​​Ideally be on the CPPE Primary Care Pathway, or willing to undertake it (exemption routes available for highly experienced pharmacists)
