About the role
We are looking for a dedicated and motivated individual to join our medicines optimisation team, helping to ensure that patients receive the right medication at the right time to improve their health outcomes. In this role, you will work across a variety of primary and community care settings, including GP surgeries, care homes, community mental health services, and learning disability services.
Your responsibilities will include assisting pharmacists with medication reviews, collecting data for audits related to medicines and patient care, supporting repeat prescribing projects aimed at reducing waste, and helping with the correct ordering, dispensing, and storage of medicines. You will also be involved in creating detailed reports and correspondence for GP surgeries outlining work undertaken, outcomes, and recommendations. There will be a requirement to cover the lead technician at the prison when required
The Medicines Optimisation Team supports patients and clinical teams across a wide variety of settings including mental health, prison and care home settings and there will be an expectation that the successful candidate will work across all sectors as required.
