About the role
Are you passionate about making a real difference in your community and helping people access the right support at the right time? This is an exciting opportunity arising from the development of locality-based Health and Wellbeing Communities. As a Community Worker, you’ll be a key point of contact within the community, helping to deliver truly person-centred support that makes a meaningful impact on people’s everyday lives.
In this varied and hands-on role, you’ll work proactively alongside a multi-agency team to promote integrated care and ensure people receive coordinated support to stay well within their own communities. You’ll take the lead in supporting individuals directly, coordinating input from partners, carrying out basic assessments, and gathering and sharing information to inform partnership discussions. Your work will also contribute to shaping and developing services within your locality, ensuring they respond to the needs of the community and support our vision of delivering the right care, at the right time, in the right place.

Things to note
An enhanced with barring police check is required for this post, the cost of which will be covered by the Isle of Man Government.
