Manx Care is seeking a compassionate, motivated and proactive Mental Health Discharge Support Worker to join our Integrated Mental Health Services team at Manannan Court.

This is an exciting opportunity to play a key role in supporting patients through one of the most important stages of their recovery journey – safe and effective discharge from hospital to community-based care. Working alongside multidisciplinary teams, patients, families, carers, and external agencies, you will help ensure seamless transitions that promote independence, recovery and wellbeing.

About the Service

Manannan Court is an acute mental health inpatient unit dedicated to supporting individuals experiencing a mental health crisis or relapse. The service comprises:

  • Harbour Suite – supporting adults of working age
  • Glen Suite – primarily supporting older adults

Our focus is on delivering high-quality treatment, care, and support that enables people to recover as quickly and safely as possible

The Role

As a Mental Health Discharge Support Worker, you will:

  • Coordinate and support discharge planning for patients and their families.
  • Work closely with multidisciplinary teams, care homes, community services, social care, housing providers, and voluntary sector organisations.
  • Help identify and overcome barriers to discharge, reducing delays and improving patient flow.
  • Support patients to understand discharge plans, follow-up arrangements, and available community resources.
  • Promote recovery, independence, relapse prevention, and social inclusion.
  • Maintain accurate electronic records and discharge-related documentation.
  • Facilitate referrals and contribute to holistic care planning and reviews.
  • Act as a key point of contact for discharge-related enquiries from patients, carers, and partner agencies.
