About the service

This service supports three gentlemen who live in their own homes, each with unique support requirements. As a support practitioner, you will be instrumental in providing assistance within our 24-hour housing support service, supporting individuals with diverse support needs, some of whom may have challenges in verbal communication.

Our dedicated team takes pride in working collaboratively to ensure the safety, well-being, and fulfilment of the individuals we support. In this role, you'll engage in 2:1 support, assisting individuals in their day-to-day lives while empowering them to pursue happiness and fulfilment, both within their homes and in the broader community.

At the heart of your duties lies the nurturing of trusting connections, serving as a foundation for assisting with practical tasks including:

  • Providing personalised support, carrying out personal care, housework, medication administration, meal preparation, financial management, and fostering companionship.
  • Collaborating with families and professionals to deliver top-quality care tailored to each individual's needs.
  • Advocating for independence and honouring the uniqueness of personal Support Plan.
  • Maintaining records of daily tasks and ensuring they are kept up to date.
  • Working cohesively within a team, fostering effective communication with colleagues, families, and the individuals receiving support.
